Request to Enrol at Wadestown Medical Practice

 

We are accepting new patients living in Wadestown / surrounding suburbs or family members of existing patients.

In order to enrol you will need to complete our enrolment form and provide us with:

• photo ID (Passport, Driving License, etc) AND

• proof of eligibility (NZ Passport, Citizenship documentation, Work/Resident Visa, Birth Certificate etc).

Please note the requirement for proof of eligibility is set by Te Whatu Ora not ourselves.

We do not usually see casual or one off appointments unless in a clinical emergency, call reception to see if we have space for a casual appointment. All casual appointments must be paid for on the day. Please note we do not have paywave facilities.

New Patient - Nurse Appointments
We require all new patients (excluding newborns) to see a nurse for a New Patient appointment before your first GP consultation. This nurse appointment is to go over your medical history and take base-line measurements, so that you and your doctor can then focus on dealing with your consultation.

We recommend you book in for your New Patient nurse appointment as soon as you are enrolled with us so it is done before you need to see a doctor. The standard nurse appointment fee applies. If you need to see a GP urgently before you can complete your enrolment and have your New Patient appointment, we can see you as a casual patient and the non-funded appointment fee will apply.
